• Your Name
• Owner Number
• Old Address
• New Address
• Last four digits of the Social Security number for verification
• Daytime phone number including area code
• E-mail address if available

The required documentation for a name change is a copy of the recorded document indicating the name change or a copy of the marriage certificate.

To update your Social Security or Tax ID information, you will need to submit a completed IRS Form W9, Request for Taxpayer Identification Number and Certification. The form can be found on the Internal Revenue Service website at www.irs.gov. Also include your owner number and old Social Security or Tax ID number, a daytime phone number including area code, and an email address if available with your information. You can also contact our office at (303) 296-9900 to request a copy of the IRS Form W-9 to be sent to you by mail.
